The Church also ministers to the sorrowing and consoles them in the funeral rites with the comforting Word of God and the Sacrament of the Eucharist."(Order of Christian Funerals, no. 4)
When the time comes, we hope to celebrate your life in faith and according to your wishes. Planning ahead is a thoughtful and loving gift to your family—so whether you are filling out this form as peace of mind for your loved ones, or you have recently lost your loved one, please take time to speak with your family and fill out the information below.

Please take your time to prayerfully review, complete, and select your preferences. Once finished, submit your form and it will automatically be sent to the Funeral Coordinator at the Parish Office. We will provide you with a copy for your records via email, which you may keep with your important papers at home.
Liturgical Guidelines
To ensure a reverent and meaningful celebration of Christian burial, please note the following:
First Reading – Select from the Old Testament
Second Reading – Select from the New Testament
Music – Must be sacred or liturgical in nature (secular music is not permitted during the Funeral Mass)
